Staging and treatment of malignant melanoma

Abstract:

The therapy of malignant melanoma��MM�� is still a tough problem up to now, most dermatologists including dermatological surgeons often lack a clear understanding about it. Excisional biopsy is recommeded for doubtful primary lesions prior to pathological examination, and when lymph node metastasis is highly suspected, sentinel lymph node biopsy should be initiated. Nowadays, the staging of MM is based on several indices, including tumor thickness, the presence of ulcer, the number and size of involved lymph nodes, and the occurrence of metastasis, which is according to the staging standard established by the American Joint Committee on Cancer in 2002. As far as the treatment of MM is concerned, primary lesions should be removed by enlarged excision, and staging-based systemic adjunctive therapy is recommended to those patients with metastasis, such as interferon-α 2b, tumor vaccines, interleukin-2, cytotoxic drugs and so on. After the operation, a close follow-up is required to guard against recurrence and development of new lesions. Futural research should be concentrated on vaccine-based immunotherapy and targeted chemotherapy.
