To report a case of acanthosis nigricans with insulin resistance accompanied by dermatitis vegetans-like lesions. The patient, an 11-year-old girl, had been obese since she was 4 years old. Physical examination showed well-marginated, thickening erythema covered with proliferative lesions on the dorsa of both hands, feet and groin, as well as dark gray hyperpigmentation on the neck, axillary fossa and groin. The patient had been treated as eczema for a long time with no obvious effect. Endocrine examination following admission indicated hyperinsulinemia and insulin resistance. The case was considered as an unusual appearance of acanthosis nigricans with insulin resistance. Neither diet-control nor exercise therapy resulted in any improvement in this patient. The lesions resolved after treating with metformin.
