The pathogenesis of anti-neutrophil cytoplasmic antibody (ANCA) associated vasculitis remains unclear at present. However, recent studies demonstrated that autoantibodies against cytoplasmic antigens of proteinase 3 (PR3) and myeloperoxidase (MPO) were closely associated with these vasculitic disorders. ANCA interacts with neutrophils and monocytes. Adherence of neutrophils and monocytes to the endothelium was followed by activation of these cells by ANCA. Activated neutrophils and monocytes released reactive oxygen intermediates, which lead to endothelial cell injury and eventually to necrotizing inflammation of small vessels.
