A 28-year-old male patient presented with hypopigmented patches on the trunk and upper limbs for 8 years. The lesions were characterized by multiple, variously sized, ill-defined, non-scaly, hypopigmented patches that were confluent around the midline trunk. Potassium-hydroxide test of skin scrapings was negative. Wood's lamp examination of the lesions revealed red follicular fluorescence. Histopathology exhibited a decrease in pigmentation in the basal layer. Under an electron microscope, fewer melanosomes were observed in lesions compared with adjuvant normal skin; melanosomes at stage ��- �� predominated in lesions, but stage �� melanosomes in adjuvant normal skin. The case was diagnosed as progressive macular hypomelanosis according to the clinical feature, results of Wood's lamp test, histopathological and electron microscopic examination. The lesions responded well to narrow-band UVB phototherapy.
